Cltrulination is a modificatlon of protiens where a nitrogen In the amino acid arginine IS replaced with an oxygen, converting it into citulline. Vimenbn, a cytoskeleton intermediate filament (IF) protein present In cells of mesenchymal orgin, including leukocytes, endothelial cells, and smooth muscle cells, IS found to be highly citrullinated at R440 and R450. Various kinds of citrullinated proteins have been detected in the joints of RA patients including mutated citrullinated vimentin (MCV). Detection of autoantibodies against MCV is part of rheumatoid arthrits (RA) test, especially in sera negative for rheumatoid factor (RF negative sera). Citrullinated vimentin has been shown to have a role in the production of anti-citrullinated protein antibodies (ACPAs). ACPAs against citrullinated proteins, such as vimentin, are considered to be highly specific markers for rheumatoid arthritis and other autoimmune diseases. The anti-MCV assay for identification of RA shows a sensitivity of 99.7%. Thus the anti-MCV detection is very important for POCT in the early detection of RA.
